Home health RN nurse case managers work with an interdisciplinary team to care for patients in the home setting. Join our amazing home health team where you have autonomy, a reasonable and fair weekend rotation, a flexible schedule, and limited on-call.
Home health RN’s provide skilled care to patients to achieve outcomes in collaboration with the patient’s provider.changes regarding patient are communicated to the following physician
Minimum Qualifications :
Associate's Degree (Required)DL - Driver's License - Department of Motor Vehicles, WCC - Wound Care Certified - National Alliance of Wound Care and Ostomy (NAWCO)
Knowledge of homecare nursing, wound consultation, Medicare regulations, JCAHO standards, home care reimbursement, and basic personal computer skills.
Knowledge in assessment, planning, implementation and evaluation of nursing care. BLS certification. Current driver's license, auto insurance and reliable transportation.
Wound Certification (WOCN) or Experience Preferred.
