Full-Time Weekday LPN – Deliver Compassionate Home Health Care with Purpose

Location : Grand Junction, CO

Schedule : Monday–Friday | 8 : 00 AM – 5 : 00 PM

Pay : $25–$35 / hour (based on experience)

What You'll Do

As a key member of our home health team, you will :

Provide skilled nursing care to patients throughout Mesa County.

Deliver services such as wound care, medication administration, catheter care, and more.

Collaborate with RNs and the care team to monitor patient progress and update care plans.

Support patients through chronic condition management and health education.

Accurately document all care provided (training in HCHB EMR included).

Travel locally for patient visits (mileage and drive time reimbursed).

What You'll Need

Active LPN license in the State of Colorado

IV certification preferred (not required)

• Wound care experience a plus!

Home health experience helpful, but not necessary-training provided

Strong communication, organizational, and clinical skills

Reliable vehicle, valid driver's license, and auto insurance
