Compact State Licensed Case Manager RN

A company is looking for a Case Management - Registered Nurse.

Key Responsibilities

Assess, plan, implement, and coordinate case management activities to evaluate and enhance member wellness

Utilize clinical tools to conduct evaluations and apply clinical judgment to address health and social barriers

Collaborate with interdisciplinary teams and stakeholders to overcome barriers and achieve health goals

Required Qualifications

Registered Nurse with an unrestricted nursing license and multi-state / compact privileges

3+ years of clinical / nursing practice experience

1+ year of medical case management, care coordination, or discharge planning experience

High-speed internet capability for remote work

3+ years of experience with personal computers and MS Office Suite applications
