Description

Responsible for the management and operation of the Department of Pharmacy in accordance with the hospitals stated philosophies goals and objectives to ensure the consistent provision of high-quality patient care.

Responsibilities

  • Job Specific Duties

Assures that the highest quality of employees is hired and appropriate licensure is maintained.

  • Coordinates the daily activities of the pharmacy department including the scheduling of necessary staff to carry out pharmacy services.
  • Receives and researches all requests from health care professionals (including but not limited to medical pharmacy nursing or remote hospital staff) for drug information and provides written / verbal response.
  • Cultivates and maintains good working relations with the Medical Staff and other department personnel.
  • Coordinates and participates in the initial training / orientation of new pharmacists and technicians.
  • Performs mandatory review of each employees performance in a timely manner.
  • Participates in the formulary review process by developing drug monographs related data collection and recommendation for submission to the Pharmacy & Therapeutics Committee.
  • Coordinates reporting of Adverse Drug Reaction reports on a quarterly basis to the Pharmacy & Therapeutics Committee.
  • Ensures compliance with State and other regulatory agencies such as the JCAHO DEA FDA etc.
  • Maintains appropriate hospital / pharmacy licensure.
  • Oversees pharmacy inventory and stock levels ensuring adequate quantities while avoiding excess amounts.
  • Oversees clinical pharmacy functions (MUE patient medication history / review pharmacokinetics).
  • Works in conjunction with all Nurse Managers to ensure efficient and accurate distribution storage and security of all pharmaceuticals in areas outside of the pharmacy department.
  • Attends and participates in all assigned hospital committees.
  • Reviews departmental systems effects changes as needed to improve services and simplify workflow.
  • Monitors workflow of the department; promotes harmonious working environment; encourages staff involvement in improvement for the department.
  • Follows policies and procedures in the seven areas of the Environment of Care to include : safety security hazardous medical / waste emergency preparedness life safety medical equipment and building utility systems.
  • Understands role in maintaining standard inventory PAR levels.
  • Understands reorder points (based on PAR levels).
  • Understands process for ordering product through wholesaler.
  • Understands process for ordering product direct from manufacturer.
  • Describes monitoring process.
  • States process for adding items to Want Book.
  • Demonstrate ability to run shelves for re-ordering product.
  • States hospital / department policy and procedure for ordering.
  • Demonstrates ability to place order using the ECHO system.
  • Demonstrates ability to check for appropriate contract and generic substitutions in the ECHO program.
  • States necessary lead times for ordering items
  • Hospital Wide Requirements
  • Observes department / hospital policies concerning smoking telephone use dress code and other rules governing conduct while at work. Follows established hospital guidelines with regards to appropriate access of information.

  • Meets hospital wide and department specific educational requirements. Attends all annual mandates / required in-services : (i.e. CPR Safety infection control) CPR current if required. Maintains a current Employee Health screening status as per hospital policy.
  • Utilizes hospital / departmental manuals appropriately. Knowledgeable of location and use of emergency response equipment. Responds to Emergency Preparedness Plans according to hospital policy.
  • Attends and participates in a minimum of 3 / 4ths of required meetings such as departmental or committee meetings. Maintains inter and intra departmental communication ex MOX.
  • Employee demonstrates an understanding of and adherence to the Code of Conduct. Employee conduct reflects the companys values and a commitment to the Code of Conduct
  • Maintains a safe and orderly work area. Cleans equipment as needed. Maintains equipment in safe and proper working condition through correct use and appropriate notification of malfunction.
  • When indicated completes occurrence reports in a timely manner.
  • Complies with and helps to enforce safety and health policies. Applies appropriate ergonomic principles to reduce the potential for job related injuries.
  • Implements all policies and procedures to identify and reduce the risks of acquiring and transmitting infections among patients employees and others.
  • Follows appropriate channels of communication.
  • Displays effective customer relations skills.
  • Identifies self-improvement / department improvement needs and seeks appropriate resources. Accepts responsibility for action and omissions.
  • Willing to adjust personal schedule to meet hospital staffing needs.
  • Reports to department on time and prepared to begin assigned duties.
  • Provides proper notification and advance notice for absence or tardiness. Takes corrective action to prevent recurrences.
  • Supports and encourages harmonious working relationships. Promotes an environment for an interdisciplinary team approach.
  • Self-motivated; utilizes effective time management. Seeks additional work as time permits.
  • Able to act independently. Seeks guidance about questionable or uncertain situations.
  • Helps assure the department runs effectively and efficiently and assists with improvement projects.
  • Achieve goals set prior year (employee development sheet item one).

  • Clearly communicates and integrates the hospitals mission vision values and goals at the department level.

  • Participates in hospital-wide strategic planning process and carries out delegated initiatives effectively.
  • Conducts and documents departmental meetings at a minimum of 10 / year or as appropriate to the department. Documentation includes follow-up on current issues.
  • Develops and implements policies and procedures that guide and support the provision of services.
  • Consistent in the administration of policies and procedures.
  • Recommends a sufficient number of qualified and competent persons to provide care or service.
  • Exhibits effective time management / organization skills including prioritizing and delegating.
  • Plans & develops an annual operating and capital equipment budget based on input from employees physicians & others.
  • Monitor financial reports identifies variances plans and implements opportunities for improvement. Assesses outcome of action plan. Completes monthly DOR variance report.
  • Determines the qualifications and competence of department personnel. Ensures and documents competency of staff including age specific competency.
  • Provides new employees with a complete orientation process that includes feedback from primary preceptors.
  • Provides timely Initial and annual performance evaluations.
  • Provides and communicates evaluation of staff performance on an on-going basis and at scheduled performance evaluations. Complete including all attachments examples specifics and goal setting with staff.
  • Recruits interviews selects and hires appropriate employees in cooperation with Human Resources. Turnover rate :
  • Plans and provides in-service training and continuing education of all persons in the department. Utilizes internal and external resources as appropriate.
  • Effectively utilizes the problem-solving procedure to resolve employee issues satisfactorily. Counsels employees when necessary and takes appropriate disciplinary action. Last available employee satisfaction scores increase or decrease.
  • Plans designs measures analyzes and improves departmental performance and maintains appropriate quality control programs. Implements on a timely basis.
  • Involves & educates employees in performance improvement activities.
  • Acts on performance issues identified by patients / family physicians quality monitors etc. Brings appropriate issues through the hospital-wide performance improvement process.
  • Identifies and supports a unit safety trainer to ensure departmental training is timely and accurate. Departmental representation at % of safety trainer meetings.
  • Ensures department specific policies / plans / practices to maintain a clean safe and risk-free environment. Departmental response to emergency preparedness drills. % code evaluation forms completed.
  • Educates staff on EOC issues including performance monitors new / updated policies & plans & participation and response to hospital-wide drills / incidents.
  • Collaborates with Risk Management to identify investigate resolve and minimize departmental occurrences.
  • Reinforces the companys values and promotes the Code of Conduct.
  • Meets leadership obligations under the Ethics and Compliance Program.
  • Quality / Patient Safety Accountable for identification of risks hazards and mitigation efforts of unsafe practices.
  • Qualifications

    Experience - 5 years as pharmacist.

    Education - Graduate of Pharmacy Program

    Special Skills - Must be highly proficient in the management skills of planning controlling coordinating and measuring departmental activities as they relate to hospital pharmacy. Licensure - Unrestricted license through Louisiana Board of Pharmacy
