A company is looking for a Remote Clinical Pharmacist in Virginia.

Key Responsibilities

Provide disease and medication management through phone and virtual consultations

Complete comprehensive medication reviews and support chronic disease management

Document all patient encounters accurately within the clinical care platform

Required Qualifications

PharmD degree from an ACPE-accredited institution

Licensed or eligible for licensure as a pharmacist in Virginia

Board Certification (BCGP, BCPS, BCACP) preferred or obtainable within 12 months

Working knowledge of chronic disease management and value-based care

Proficient in Google Workspace and clinical software documentation
