Advocatecare Social Worker
The AdvocateCare Center is an innovative model that drives patient engagement, improves health outcomes, and manages medical costs in the older adult population.
The AdvocateCare Center (ACC) team is a cross disciplinary team caring for patients with complex medical issues. Together this team is responsible for providing and coordinating care for an intimate panel of patients.
This creates an opportunity to spend more time with patients, build deeper relationships, and to better execute / coordinate care plans.
Panel management : This cross-disciplinary care team directs and provides patient care for a panel of patients with a broad range of clinical presentations throughout the care continuum.
Complex, chronic and geriatric patients : This team will focus on a population of high-risk, high-need patients who are (or are predicted to be) in need of focused, managed health care.
Care coordination : Provides comprehensive, coordinated and integrated care to all patients at the ACC and across all health-related interactions a patient has with the health care system.
Team management and coordination : Partners in a consultative and collegial manner as a member of a multidisciplinary team, which include Physicians, APNs, Patient navigators, social workers, behavioral health workers, dieticians, and other professionals.
The masters prepared social workers provides treatment of both the traditional psychiatric patient population as well as the integrated medical- behavioral patient population through both in person, telephonic and telehealth consultations.
Major Responsibilities :
- Direct Service : Provides crisis intervention, behavioral health assessment, psychosocial support, telephone screening, and referral services.
- Recordkeeping : Completes various clinical assessments in accordance with team policies and procedures. Records reflect quality clinical care and adhere to Accrediting Organization standards.
- Clinical and Professional Practice
- Team and Training Activities : Actively participates in team activities in the department, larger hospital and in the community in ways that make a positive contribution to team operations.
Licensure :
Social Worker, Clinical
Education / Experience Required :
MA or MSW, or equivalent. Two years' experience working in crisis work and / or hospital environment. Knowledge of Illinois State law regarding confidentiality, mental health code and federal substance abuse codes.
Knowledge of behavioral health resources. Knowledge of DSM-IV diagnostic categories. This position will need to recognize the needs and behaviors of a variety of age groups of patients treated, i.
e., normal patterns of adolescent behavior and look for signs or normal aging, prior experience with various age groups is preferred.
Knowledge, Skills & Abilities Required :
Basic computer skills. Organization, time management, communication, and documentation skills. Ability to deal with conflict.
Effective interpersonal skills and ability to remain calm under stressful situations and exercise appropriate intervention.
Current licensure in the State of Illinois as a Social Worker (LSW) within 6 months. CPR Certification.
Physical Requirements and Working Conditions :
- Resiliency and flexibility to deal with a changing environment. *Ability to work under stressful conditions and in difficult situations.
- May be exposed to patients that potentially could be violent; has knowledge of techniques to de-escalate aggressive behavior.
- Flexibility and openness to working any crisis shift. *Ability to flex hours and days to meet needs of hospital / department.
