Home Health: Physical Therapist (PRN/Part Time) - Fort Myers

Responsibilities:
- Conduct comprehensive evaluations of patients' physical conditions, including assessment of mobility, balance, strength, and pain levels
- Develop personalized treatment plans based on patients' specific needs and goals
- Implement evidence-based therapeutic interventions to improve patients' physical function and reduce pain
- Provide hands-on physical therapy to patients in their homes, ensuring their safety and comfort
- Monitor patients' progress and adjust treatment plans as necessary
- Collaborate with other healthcare professionals, such as physicians and nurses, to ensure coordinated and effective care
- Educate patients and their families on proper techniques for exercises and activities to promote independence and recovery
- Maintain accurate and up-to-date documentation of patient evaluations, progress, and treatment plans
- Stay current with advancements in physical therapy through continuing education and professional development opportunities
Requirements:
- Current state license as a Physical Therapist in Florida.
- A Minimum of 1 year of experience in home care, or a related field
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking abilities
- Valid driver's license and reliable transportation to travel to patients' homes.
