Position Summary :
Provide flexible, on-call coverage for open shifts and staff absences across New Heights service locations. Support adults with diverse needs in their homes and community to promote independence, safety, and inclusion in alignment with each person’s Individual Plan of Service (IPOS).
Key Responsibilities :
- Follow each person’s IPOS and any safety / behavior plans; use least-restrictive, trauma-informed approaches
- Provide non-medical support with daily routines : meals, light cleaning, dressing, hygiene (as authorized)
- Assist with community participation : errands, appointments, recreation, social activities
- Adapt quickly to different service locations and individual support needs when covering shifts
- Provide transportation per policy (when authorized)
- Document services accurately and on time in the approved system (e.g., EVV / HHAeXchange)
Complete incident reports as required
Communicate coverage updates and observations to the Administrator / Program Director; collaborate with the care teamProtect privacy and follow Recipient Rights, HIPAA, infection control, and all federal / state, and agency policiesQualifications :
High school diploma or GED; at least 18 years oldOne year of community / in-home or human services experience with adults preferredAvailable to work flexibly across all shifts (days, evenings, overnights, weekends, holidays)Valid Michigan driver’s license, reliable transportation, and auto insurance if drivingAble to pass background checks and complete required training (Recipient Rights within 30 days, CPR / First Aid, etc.)Professionalism, clear communication, reliability, and basic documentation skillsWork Environment :
Services occur in adults’ homes and community locations across Oakland County; travel between sites may be frequentSchedules vary based on authorized hours and coverage needs; assignments may change with short noticeRole involves light to moderate physical activity (household tasks, community walking) and safe assistance with mobility as authorizedCompensation
