Intake Coordinator - Home Health - Lynwood, WA

The intake coordinator receives and analyzes incoming referral and order calls. Responsibilities Answers phones and appropriately triages phone calls to Intake Team. Effectively communicates patient / referral information to appropriate Intake staff and branches. Obtains demographic and clinical information from clients, physicians office and other referral sources to compile referrals. Coordinates communication of referral information with Intake Specialist RN. Maintains a call log for all incoming calls. Faxes signed referrals to appropriate county / parish offices, DME, private duty, etc. Verifies new referring physician licenses on referrals taken in department. Researches coverage or pay source for each referral. Verifies commercial policies and HIQA. Obtains authorization for visits from insurance or Medicaid on EPSDT patients as appropriate.
