Currently serving the following cities in Macomb County : Roseville, Shelby Twp, Warren, Sterling Heights, & Clinton Township (subject to change as recipient needs change).

Activities (most critical to least critical) Performs physical therapy evaluations of client functioning levels, plans short- and long- term goals toward improving / restoring independence and physical function.

Applies diagnostic and prognostic muscle, nerve, joint and functional ability tests. Implements treatment to clients to relieve pain and / or restore function to maintain maximum performance.

Maintains clinical documentation and progress notes. Submits all required paperwork in a timely manner according to ExpertCare policy.

Periodically reports to the physician on the client’s treatment, response and profess and assists physician in determining any necessary changes in the treatment program.

Instructs clients and their families in the use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, etc.

Instructs and familiarizes, as necessary, the client and pertinent family members with all aspects of the client’s total physical therapy program.

Attends therapy staff meetings and in-service presentations. Participates in staff and referring organization development activities Prepares an assessment of the client’s condition and periodic evaluations and summaries, develops a plan of care and problem list, and writes clinical / progress notes on the date of the visit and incorporates same in the client record.

Participates in Annual Person-Centered Planning meetings and Periodic Status Review meetings to provide insight and update on treatment planning and implementation.

Assumes responsibility for supervising the Physical Therapy Assistant at least every 30 days.
