The role involves providing skilled physical therapy services to a diverse patient population in their homes, promoting recovery, independence, and overall well-being. As a Home Health PTA, you will manage an autonomous caseload, traveling between patient residences within assigned territories including Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont, Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, and Union City. You will work closely with a supervising Physical Therapist to implement individualized treatment plans, educate patients and caregivers, and meticulously document progress. Candidates have the option of a guaranteed hours model, requiring a commitment to productivity and flexibility, or a non-guaranteed, visit-based model offering more autonomy. Both options require a willingness to provide care to all patients, including those who are COVID-19 positive or Persons Under Investigation (PUI). Key Responsibilities Deliver skilled physical therapy assistant interventions in accordance with the established plan of care developed by a supervising Physical Therapist. Monitor and assess patient progress, reporting any significant changes or concerns to the supervising Physical Therapist. Educate patients, families, and caregivers on therapeutic exercises, adaptive equipment, and safety measures to promote independence and prevent injury. Maintain accurate, complete, and timely documentation of all patient visits, treatments, and communications in compliance with regulatory standards. Manage an assigned caseload efficiently, scheduling visits and prioritizing patient needs within the designated service area. Adhere to all professional, ethical, and legal standards of practice for Physical Therapy Assistants in California. Demonstrate willingness to travel extensively within the assigned territory and occasionally outside the primary service area to meet caseload demands. Required Qualifications Minimum of one (1) year of professional experience as a Physical Therapy Assistant. Ability to travel independently and reliably throughout the assigned service area. Valid Driver's License, current Automobile Insurance, and current Vehicle Registration. Ability to meet the physical demands required for a home health role, including lifting, bending, and prolonged periods of standing and walking.
