Stroke Recovery Specialist – Restore Function for LA Seniors After Stroke Company: All Seniors Foundation Location: Los Angeles, CA About Us: All Seniors Foundation is a 501(c)(3) non‑profit committed to improving the quality of life for older adults across Los Angeles County. we provide medical care, in‑home support, hospice and palliative care, cancer navigation, and 200+ other free services to qualifying seniors. Every service is delivered at no cost, regardless of insurance status. Role Overview: As Stroke Recovery Specialist (Neuro Rehab Nurse), you will lead post‑stroke rehabilitation programs for seniors at home, coordinating with PT, OT, SLP, and primary care to optimize recovery and prevent secondary stroke. You will educate patients and caregivers on stroke‑prevention lifestyle changes. Key Responsibilities: Conduct comprehensive stroke recovery assessments using NIH Stroke Scale and functional measures Develop individualized rehabilitation plans coordinating with PT, OT, SLP, and primary care Educate patients and families on secondary stroke prevention (BP, A‑fib, diet, exercise) Monitor neurological status, vital signs, and medication adherence in the home setting Coordinate with neurology, cardiology, and pharmacy on integrated care plans Document encounters in EHR and contribute to stroke quality metrics Qualifications: Current CA RN license + BSN preferred 2+ years stroke, neuro, or rehab nursing experience CRRN (Certified Rehabilitation Registered Nurse) or SCRN (Stroke Certified Registered Nurse) preferred ACLS + NIHSS certification Strong patient and family education skills What we Offer: Competitive salary ($75,000–$105,000) Mix of home visits, telehealth, and clinic coordination CRRN/SCRN certification fees + neuro CEUs funded Comprehensive benefits including health, dental, vision, 401(k) Mileage reimbursement; manageable caseload What to Expect on the Job Your week mixes in‑home stroke assessments, family education sessions, multidisciplinary case conferences, and care planning.
