Work Environment

This position is based onsite, directly in patients' homes across Lakeland, Florida. Our dedicated home care services allow our therapists to provide personalized care in a comfortable environment. You will interact closely with patients in their everyday surroundings, allowing for tailored treatment plans that consider their unique needs and lifestyles. Key Responsibilities

• Conduct thorough assessments of patients to develop effective, individualized treatment plans aimed at improving mobility, strength, and overall function.
• Implement a variety of therapeutic interventions including active and passive exercises, muscle re-education, manual therapy techniques such as massage and stretching, and gait training.
• Educate patients and their families on care plans and proper techniques for self-management of their conditions.
• Collaborate with other healthcare team members to provide comprehensive care, ensuring that all aspects of patient treatment are aligned with the organization's best practices.
• Track and document all patient progress in line with clinical protocols and participate in case reviews to enhance treatment strategies.

Patient Population / Client Focus

The ideal candidate will work with a diverse patient population, primarily consisting of individuals recovering from orthopedic surgeries, injuries, or chronic conditions. This role is crucial in helping patients regain function, alleviate pain, and improve their day-to-day activities, contributing significantly to their recovery and long-term health outcomes.

Qualifications

• Minimum of a Bachelor’s Degree in Physical Therapy from an accredited institution.
• Possession of an active State Physical Therapy license.
• At least 1 year of professional experience in orthopedics or related settings.
• Exceptional clinical assessment skills and the ability to develop effective treatment plans.

Preferred Experience

• Preference will be given to candidates with board certification in Orthopedic Physical Therapy.
• Experience in home care or outpatient settings is valued.
