SYNERGY HomeCare of Central IL is looking for compassionate, reliable, and dedicated caregivers to provide quality care to clients in the comfort of their own homes.

Job Responsibilities

As a caregiver, you will help clients maintain their independence, dignity, and quality of life by providing personalized assistance with everyday activities.

• Assist clients with bathing, dressing, grooming, toileting, and personal hygiene.

• Provide assistance with walking, mobility, positioning, and safe transfers.

• Assist with feeding and other activities of daily living (ADLs).

• Provide incontinence care and assistance with personal needs.

• Provide companionship, conversation, and emotional support.

• Engage clients in activities that promote mental, emotional, and social well-being.

• Encourage clients to remain as active and independent as possible.

• Prepare meals and assist with feeding as needed.

• Perform light housekeeping, laundry, dishes, and general household organization.

• Assist with grocery shopping, errands, and other essential daily tasks.

• Help maintain a clean, comfortable, and safe home environment.

• Provide medication reminders in accordance with the client's care plan.

• Follow individualized care plans and company procedures.

• Observe and report changes in a client's physical condition, behavior, or overall well-being.

• Communicate professionally with clients, their representatives, and the SYNERGY HomeCare office team.

• Accurately document services provided during each shift.

• Report concerns, incidents, and potential safety risks promptly.

• Provide transportation or accompany clients to appointments and errands when assigned and authorized.

Qualifications and Requirements

• Must be at least 18 years old, or 17 years old with a current, verifiable Illinois CNA certification, subject to applicable minor-work restrictions.

• Must possess a valid driver's license.

• Must have a reliable vehicle.

• Must maintain valid automobile insurance.

• Must complete the required Illinois Department of Public Health (IDPH) background check and meet applicable state employment requirements.

• Must be able to lift at least 25 pounds and safely assist clients with physical care needs.

• Must be able to work independently with minimal supervision.

• Must demonstrate strong organizational and time-management skills.

• Must be able to accurately document services and complete required care records.

• Must have strong verbal and written communication skills.

• Must be comfortable working with individuals of varying personalities, backgrounds, and care needs.

• Must demonstrate compassion, patience, dependability, and professionalism.

• Must maintain client confidentiality and appropriate professional boundaries.

• Must follow established safety procedures, infection control practices, and company policies.

• Must be able to recognize and promptly report client concerns, incidents, suspected abuse or neglect, and changes in condition.

• Must successfully complete required orientation, training, and competency evaluations.

• Previous caregiving experience is preferred but not required.

• Must have a positive attitude, willingness to learn, and a genuine passion for helping others.
