A growing, physician-led post-acute care organization is seeking a dedicated Physician to provide primary care services within long-term care communities across Pennsylvania.

Clinical Responsibilities\n

• Conduct admissions and new patient evaluations

\n • Perform routine follow-ups and regulatory visits
\n • Collaborate with NPs, nursing staff, therapists, and facility leadership
\n • Communicate with families and caregivers
\n • Document encounters using a company-issued mobile device

The focus is on quality, coordination, and patient-centered care within a structured post-acute environment.
