Who We Are :
Elliot Health System’s Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) provide individual and group-based treatment of co-occurring substance use and mental health disorders.
PHP and IOP are for individuals who need care that is more intensive than individual outpatient programs but not as intensive as residential treatment.
Depending on their role, staff can provide case coordination, assist participants in accessing community-based services, provide medication management, evaluate participants for medical issues, and more.
The teams coordinate closely with primary care providers on a biweekly basis to ensure that all aspects of patients’ wellbeing and health are addressed appropriately.
About the Job :
Provides psychiatric assessment and therapy to patients and their families in a co-occurring Partial Hospitalization setting to maximize emotional, social and physical well-being of program participants.
Working with a collaborative, multidisciplinary team, coordinates patient care through a continuum of care facilitating participant's recovery.
Works closely with the Hillsborough County North Drug Court.
What You’ll Do :
Individual Evaluation and Assessment : Provides psychiatric assessment of individuals. Based on evaluation, makes appropriate referrals (both interoffice and interagency).
Serves the best interests of patient and exercises independent professional judgment when rendering care.
- Counseling : Provides individual, group, and / or family therapy to reach therapeutic goals.
- Documentation of Services : Maintains ongoing documentation of patient interventions. Documentation must be completed within 24-hours of the service date.
This includes documentation of initial psychiatric assessment, treatment plans, and therapy sessions, as well as any other patient interaction, including phone calls.
- Liaison Functions : Represents the practice in the community. Participates in marketing, education, and other activities to promote and enhance the public image of Elliot Health System.
- Supervision / Collaboration : Participates in ongoing peer supervision and collaboration so as to maintain licensure; receives clinical input from peers.
General oversight and clinical supervision will also be provided by the PHP NP and Director of Substance Use Services.
Education : Participates in planning, developing, implementing, and evaluating education programs, support, and services offered by Elliot Behavioral Health Services.
This will include providing consultation and / or supervision to student interns.
Credentialing : Submits credentialing and re-credentialing information to Medical Staff Affairs and Credentialing Specialist within seventy-two (72) hours of receipt of applications.
Who You Are :
Education : Master's in Mental Health Counseling, Social Work, or related degree required.
Experience : Experience in the field of mental health counseling working with individuals living with alcohol or drug addiction.
Experience working with diverse client populations such as cultural and / or religious minorities, co-occurring disorders, and socio-economically disadvantaged clients.
Certification / Licensure : Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC) preferred or license eligible.
Software / Hardware : EPIC and Microsoft Office.
