Position Summary :
The Supervisor, Community Health Worker is primarily responsible for guiding, monitoring, and coaching team members to ensure compliance with standards of practice and assure the delivery of quality care service.
The supervisor will support the team in day-to-day management to achieve goals and objectives at employee, departmental, and organizational level.
Actively maintains a caseload providing supportive assistance to a diverse patient population to identify barriers to self-care and healing, decision making, and communication in navigating the healthcare system.
Key Responsibilities :
- Conduct and participate in the recruitment, hiring, and onboarding process of new community health workers as needed
- Demonstrates a high standard of ethical behavior, professionalism, sensitivity, and confidentiality
- Ability to create, update, and implement trainings, as well as identify professional development opportunities for new and existing Community Health Workers
- Coach and support Community Health Workers with understanding their role and responsibilities, resolve questions, and serve as a liaison between staff and upper management to facilitate communication
- Lead team meetings and represent the CHW Team in organizational meetings as needed
- Assist in conducting quality assurance audits and perform one-on-one meetings with team members to increase employee engagement and improve employee satisfaction and performance
- Maintain a minimum caseload of engaged members as a Community Health Worker
- Provide non-clinical interventions and serves as the member’s navigator and advocate for needed mental and behavioral health services and community resources
- Identify community resources that support the needs of Equality Health’s member population and facilitate integration into Equality Health’s CBO Network
- Facilitate Social Determinant of Health (SDOH) screenings and interventions to address gaps in care
- Assist members with eligibility and enrollment for health insurance and special programs
- Support the members’ individualized recovery action plans, crisis plans, and self-management plans
- Actively link members to behavioral health, primary care, and community services
- Communicate care path with caregiver(s) and family with patient consent
- Engage member and family in a culturally competent manner
- Travel up to 90% within the assigned state, via personal automobile; occasional overnight stays required
Required Skills & Qualifications :
- High-school diploma or GED equivalent; or equivalent experience AND a minimum of four years of work experience in a related position
- Minimum one year of people management experience
- EMR documentation experience
- Bilingual able to speak, read and write clearly and concisely in English and Spanish, conveying complex or technical information in a manner that others can understand
- Experience working with diverse and under served populations with a strong understanding of multicultural issues.
- Familiarity with health issues in the minority population and social determinants of health
- Excellent verbal, written, and interpersonal communication skills; highly collaborative team approach to work.
- Proficient using Microsoft Office applications and web-based technologies
- Must have current driver’s license and be able to pass a DMV background check
- Experience with and comfortable using keyboards, laptops, tablets, and other electronic and portable devices for documentation and telehealth visits.
- Certified as a CHW or eligible to obtain CHW certification within one year in the role
Preferred Skills & Qualifications :
- Associate degree in social science or a health-related field is preferred

Physical Requirements :
- Comfortable and able to travel by personal automobile to multiple community locations each workday
- Comfortable using keyboards, mouse, tablet, and other electronic and portable devices for documentation
- Walking, standing, sitting, and lifting 15 lbs. frequently
- Must be able to perform patient assessments in private homes, which may have allergens or odors such as pet dander or tobacco smoke
