Ahava Therapy Services is hiring a Full-Time Occupational Therapist (OT) to serve Maplewood Healthcare and Laurelwood Healthcare in Jackson, TN. These skilled nursing facilities offer both short-term rehab and long-term care, supported by a strong, welcoming In-House Rehab team and outstanding leadership.
Responsibilities
- ️ Review available resident information and assess how it impacts evaluation and treatment
- ️ Conduct and document comprehensive functional assessments using standardized procedures
- ️ Develop and implement individualized treatment plans with achievable functional goals
- ️ Maintain timely, accurate documentation that meets state, federal, and company standards
- ️ Enter treatment and progress notes following each session
- ️ Revise treatment plans as resident needs evolve
- ️ Document treatment outcomes in clear, concise discharge summaries
- ️ Provide instruction and guidance to COTAs, ensuring proper supervision
- ️ Educate residents and caregivers on therapeutic procedures to continue after discharge
- ️ Partner with nursing, families, and interdisciplinary teams to maximize functional outcomes
- ️ Participate in care conferences, team meetings, and in-service programs
- ️ Access continuing education to maintain clinical competency
- ️ Adhere to company, state, and federal regulations and quality standards
- ️ Support quality improvement initiatives and respond appropriately to audit findings
- ️ Understand payer rules and allocate resources appropriately
- ️ Maintain expected productivity standards
- ️ Demonstrate professionalism, teamwork, and willingness to help colleagues
Qualifications
️ Bachelor’s or Master’s degree in Occupational Therapy️ Current OT license or eligibility in the state of Tennessee️ Current BLS / CPR certification (or obtained during orientation)Full-Time Benefits
