Part Time Licensed Practical Nurse / LPN (Home Health)

• Provide skilled nursing care to patients in their homes under the supervision of an RN or physician\n
• Monitor patient health status and report changes in condition\n
• Administer medications and treatments as prescribed\n
• Perform wound care, catheter care, and other clinical procedures\n
• Educate patients and families on disease management and care plans\n
• Document patient visits and maintain accurate medical records\n
• Collaborate with interdisciplinary team to ensure quality patient care\n\nQualifications : \n
• Graduate of an accredited practical nurse or vocational nursing program (LVN / LPN)\n
• Minimum one (1) year of experience, preferably in home care or med / surg\n
• Current licensure in the state and CPR / BLS certification\n
• Strong communication and problem-solving skills\n
• Ability to meet physical demands of the job\n
