Compact RN Care Manager

A company is looking for a Care Manager (RN).

Key Responsibilities

Develop and facilitate complex care management activities for members with physical needs

Educate members on healthcare options and coordinate care with providers and community resources

Monitor member progress and update care plans as necessary to meet individual goals

Required Qualifications and Education

Degree from an Accredited School of Nursing or Bachelor's degree in Nursing

2 - 4 years of related experience

RN - Registered Nurse State Licensure or Compact State Licensure required

Compact Nursing License required

Completion of 4-5 week virtual training program
