The Case Management Assistant responsibilities include :
- Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care
- Responsible for assisting with distributing regulatory documents to identified patients
- Educating patients about the appeal process related to their discharge
- Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service
- Requires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives
