Texas Licensed Senior Care Manager

A company is looking for a Senior LTSS Service Care Manager (RN).

Key Responsibilities

Perform care management duties for complex / high acuity populations

Develop personalized care plans and educate members and families on services

Monitor care plans and member progress, adjusting as necessary

Required Qualifications

RN - Registered Nurse - State Licensure and / or Compact State Licensure required

Resource Utilization Group (RUG) certification required for Superior

Bachelor's degree in Nursing preferred

4-6 years of related experience
