Job Title : Home Health LVN Case Manager

Position Overview :

We are seeking a dedicated and experienced Licensed Vocational Nurse (LVN) to join our team as a Home Health LVN Case Manager.

In this role, you will be responsible for coordinating and managing the care of patients in a home health setting. The ideal candidate will have excellent clinical skills, strong communication abilities, and a compassionate approach to patient care.

Responsibilities :

1. Patient Assessment and Care Planning :

• Conduct comprehensive assessments of patients' health status.
• Develop individualized care plans based on assessment findings, in collaboration with the interdisciplinary team.

2. Coordination of Care :

• Coordinate and manage the overall care of assigned patients.
• Collaborate with physicians, therapists, and other healthcare professionals to ensure a seamless and integrated care experience.
• Monitor and adjust care plans as needed to meet the evolving needs of patients.

3. Patient Education :

• Educate patients and their families on the management of health conditions, medications, and treatment plans.
• Provide guidance on preventive measures and wellness strategies.

4. Clinical Documentation :

• Maintain accurate and up-to-date patient records, documenting assessments, care plans, interventions, and outcomes.
• Ensure compliance with regulatory standards and agency policies.

5. Communication :

• Communicate effectively with patients, families, and the interdisciplinary team.
• Provide timely and relevant updates to physicians and other healthcare professionals involved in patient care.

6. Monitoring and Evaluation :

• Regularly assess patient progress and adjust care plans accordingly.
• Participate in case conferences and team meetings to review patient outcomes and address any challenges.

7. Quality Improvement :

• Participate in quality improvement initiatives to enhance the delivery of home health services.
• Contribute to the development and implementation of best practices.

Qualifications :

• Current and valid LVN license in the state of practice.
• Minimum of X years of experience in home health or a related healthcare setting.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a collaborative team.
• Knowledge of Medicare / Medicaid regulations and home health accreditation standards.
