Job Title : Patient Care Coordinator / Patient Services Representative

Job Type : Full Time (Monday to Friday, 8 : 00am to 5 : 00pm)
Compensation : $18 $22 per hour depending on experience
Location : Golden Valley, MN

has the perfect opportunity for you to continue your career growth! Our team of Patient Care Coordinators are instrumental in providing a seamless patient experience and making sure all documentation and information is collected and organized appropriately.
Successful candidates will have a passion for providing a high level of customer service to home care patients. Additionally, selected candidates will have had prior customer service experience in a fast-paced environment.
Patient Care Coordinator Responsibilities :
- Send and track required face-to-face documentation for Physicians to sign and return
- Collect information for currently hospitalized clients : H&P (History & Physical), ROC (Resumption of Care), MD orders, etc.
- Conduct pre-admit calls to verify client information, as well as five-day and discharge follow-up calls
- Track hospitalized clients and notify Community Liaisons of discharges, in order to resume providing care
Patient Care Coordinator Qualifications :
- High school graduate; college degree preferred
- Knowledge of medical terminology is strongly preferred
- Minimum of one (1) year of experience in an office or health care setting preferred
- Strong verbal, written, and telephonic communication skills
- Demonstrated ability to type at least 50 words per minute
