POSITION TITLE : Director of Resident Care and Service Coordination

REPORTS TO : EXECUTIVE DIRECTOR

PURPOSE : Responsible for the overall health care management of the residence while overseeing the Residents’ activities of daily living.

The Director of Resident Care and Service Coordination is responsible for all healthcare operations including : safety standards, quality assurance, infection control, admissions, service referrals and special projects.

STATUS : Salary / Exempt

POSITION REQUIREMENTS :

  • Associates degree or higher in a relevant field such as human service management, housing management, social work or nursing. RN preferred.
  • Massachusetts driver’s license with a safe driving history.
  • At least 5 years of experience in community health, human services or assisted living.
  • Excellent organizational, communication and social skills.
  • Ability to work in a team.
  • The ability to develop and maintain facility census, with a strong understanding of assisted living, Medicaid, Medicare, SWH programs.
  • Ability to deal tactfully with all residents, family members, visitors, government agencies / personnel and general public.
  • Additional requirements at the discretion of the Executive Director.
  • Must complete 3-day orientation on Assisted Living and other required topics according to EOEA’s Assisted Living Regulation 651CMR section 12.07.
  • Must complete a minimum of 15 hours per year of ongoing education and training.

GENERAL PHYSICAL REQUIREMENTS :

Includes, but not limited to :

  • Frequent bending for filing.
  • Employee may be required to sit or stand stationary for a period of time while completing office functions, i.e. 20 -30 minutes.
  • Ability to lift up to 20 -30 lbs., i.e. charts, books, paper, etc.
  • Ability to sit at a desk for extended periods of time (encouraged to change position frequently).
  • Able to tolerate walking throughout the facility on cement and carpeted floors.
  • An annual physical exam and negative Mantoux test (or chest X-ray).
  • Able to tolerate frequent stair climbing to freely & easily get to any of the facility levels.

ESSENTIAL DUTIES AND RESPONSIBILITIES :

  • Serves the Resident’s as a member of the direct care team, while leading staff.
  • Develops and maintains good working relationships with staff, residents and their families.
  • Serves as the first line for mitigating concerns with meeting the resident's or family expectations.
  • Develops and provides individualized care according to resident needs and plan of care.
  • Assists with new employee selection and orientation. Evaluates the skills of employees and makes recommendations to Executive Director regarding the employee performance.
  • Communicates new programs, interventions and new procedures with the staff.
  • Maintains all resident records in full compliance with EOEA regulations.
  • Oversees and conducts medication passes.
  • Coordinates resident care with physicians, pharmacies, certified agencies and all health care vendors.
  • Oversees all residence staff and performs assessments and in-service training.
  • Ensures resident needs are met by coordination and monitoring measures utilizing restorative services, activities and related support services allowing for respect and dignity of each resident.
  • Puts Resident service first. Ensures that Residents and family receive the highest quality of service in a caring and compassionate atmosphere and recognizes individual’s needs and rights.
  • Must respect the principles of resident rights, confidentiality, EEO and ADA.
  • Other duties may be assigned.

Resident Care

  • Conducts daily rounds checking on resident and unit cleanliness.
  • Oversees ongoing wellness programs to Resident’s and staff.
  • Implements services for Resident move-in, helps Residents with orientation to

the assisted living community / staff and with their overall adjustment to the assisted living community.

  • Informs emergency contact and appropriate staff of changes in a resident’s condition and needs.
  • Responsible for conducting and / or delegating the SAMM training for all appropriate staff and conducting any necessary disciplinary action for non-compliance with SAMM policies and procedures including regular review of documentation.
  • Ensures all medications are available and residents receive their medications as ordered.
  • Counsels the Resident’s and their family members in meeting residency standards and health care / psychosocial needs.
  • Concerns him / herself with the safety of all community residents in order to

minimize the potential for fire and accidents. Also ensures that the location adheres to the legal, safety, health, fire and sanitation codes by being familiar with his or her role in carrying out the site’s fire, safety, and disaster plans.

  • Ensures that all areas of documentation and assessment are compliant by conducting and / or reviewing regular audits.
  • Communicates and collaborates with outside providers to ensure all personnel involved with resident care are kept informed, care is consistent, predictable and organized to avoid unnecessary confusion, duplication or overlap of responsibilities related to Resident’s health and well-being.
  • Performs triage of Resident with sick complaint and refers to appropriate MD office and / or ER with completed Resident fact sheet with reason for visit.
  • Participates in agreement meetings annually and assures documentation is updated.

Admissions

  • Serves as the primary contact for new admission inquiries.
  • Completes pre-admission assessment and determination of financial eligibility in collaboration with the Special Care Residence Manager including mental health history and psychosocial issues.
  • Assists the Executive Director in implementing marketing plans including networking with potential referral sources such as physicians, hospitals, case managers and other community representatives.
  • Educates and assists potential residents and their families with Admissions process, including, but not limited to reviewing lease, rights and disclosures, and ensuring all necessary admission paperwork is completed and maintained.
  • Provides tours of the facility, answers all admission calls, and verifies all insurance and financial information.
  • Represents the Facility when working with other agencies, departments, and other organizations.

Case Management

  • Ensures effective case management and follow-up practices.
  • Assists residents and families with financial, social security, Medicare and insurance issues as requested.
  • Assists Executive Director with regard to policies and procedures, resident safety, ability to age in place, and discharge planning as needed.
  • Attends and completes all annual in-services as required by EOEA.
  • Participates in multidisciplinary care plan meetings as needed, identifying problems, setting goals and reviewing outcomes.
  • Documents all case management activities promptly in Resident records.
  • Assists in developing a Comprehensive Service Plan and review every 6 months.
  • Reviews Initial Assessments after 30 days and at a minimum of every 6 months thereafter.
  • Oversees the completion of Introductory Visits on all new residents and after a significant change.
  • Completes all required documentation within regulatory guidelines.
  • Provides information and referral as needed for Residents.
  • Provides outreach and remains updated on community resources.
  • Provides and / or arrange in-services to assist staff in understanding assisted living, Resident’s rights, psychosocial issues, mental health, disabilities, dementia care, and MR.
  • Respects the confidentiality and privacy of Residents in and out of building.
  • Reports incidents within appropriate time to the Executive Director, and State agency as required by regulations.

Employee Management

  • Supervises the performance of the licensed and non-licensed nursing staff. Initiates the disciplinary process as outlined in the Rubin Home Assisted Living employee handbook.
  • Attends conferences with other professionals and paraprofessionals to assure current standards of practice are maintained within the facility.
