Key Responsibilities :

• Conduct comprehensive patient assessments and create care plans
• Deliver quality home health care, including wound care, IV therapy, and medication administration
• Complete OASIS documentation accurately and efficiently using Homecare Homebase
• Educate patients and families on home care management
• Rotate on-call duties as required
