We are hiring CANS Assessors to support a statewide initiative aimed at evaluating and enhancing services for youth (ages 21 and under) with IDD. In this role, you’ll conduct in-person assessments using the Child and Adolescent Needs and Strengths (CANS) tool in home and community-based settings.

Role : Adolescence IDD Assessor (Capital)

What You’ll Do
Conduct in-person CANS assessments in homes and community settings.
Travel throughout the NYC region to meet with youth, families, and caregivers.
Use a person-centered approach to evaluate strengths, needs, and developmental concerns.
Gather information from caregivers, service providers, and relevant records to complete the assessment.
Accurately document findings in state-mandated systems while meeting compliance standards.
Coordinate language access services as needed.
Maintain productivity and quality standards defined by the program.
What You Need
Education : Bachelor’s degree in a human services field (required)
Experience :
Minimum 1 year working directly with children and / or adolescents with IDD (required)
Experience with assessments or the CANS tool (preferred)
Skills :
Strong communication, decision-making, and documentation skills
Ability to work independently and travel within the region
Proficiency in Microsoft Word, email, and web-based platforms
Bilingual (English / Spanish) strongly preferred
Other Requirements :
Valid NYS Driver’s License or NYS ID card
Willingness to travel for in-person visits
