Summary of the Position : Intensive Family Coordinator provides a professional level of case management services as well as direct advocacy services to a caseload of families.
- Intensive Family Coordinator will be responsible for completing family assessments, recruiting foster parents, completing home studies for the referring authority, locating family resources, and creating individualized service plans.
- Case management services including intakes, safety planning, team meetings, ISP development 24 / 7 support, case contacts, and case record documentation in keeping with state regulations.

Qualifications / Requirements : High School Diploma or GED is required.
- Experience with community work and at least one (1) year experience working in human services field.
- Experience in the provision of home and community based services.
Bi-Lingual / Spanish Speaking is a plus!
