About The Position

Summary : The Licensed Master Social Worker provides emotional support, counseling, and resource coordination to patients and families. They help clients cope with social, emotional, and financial challenges related to illness, hospitalization, or life changes.

Responsibilities :

  • Provide counseling and crisis intervention.
  • Develop care plans and coordinate community resources or services.
  • Collaborate with healthcare teams to support patient care and discharge planning.
  • Maintain accurate and confidential documentation.

  • 1 - Year experience Required,
  • Epic experience Preferred
  • BLS Certification,
  • LMSW or LCSW,
