Florida Licensed Care Manager (RN)

A company is looking for a Care Manager (RN) - Child Welfare.

Key Responsibilities

Evaluate member needs and facilitate care management activities to achieve optimal healthcare outcomes

Develop and monitor personalized care plans in collaboration with providers and community resources

Provide education and resource support to members and their families regarding healthcare options and services

Required Qualifications

Degree from an Accredited School of Nursing or Bachelor's degree in Nursing

2 - 4 years of related experience in pediatrics

Florida RN license or Compact State Licensure required

Experience in care management and knowledge of social determinants of health

Ability to comply with state, federal, and third-party payer regulations
