Position Title : Care Manager

Work Location : Remote (Alachua, Citrus, Lake, and Marion)

Assignment Duration : 3 months (Possibility to extend or convert?)

Work Schedule : 8am-5pm ET, no OT

Work Arrangement : Member-Facing

Candidate Requirements

Education / Certification

Required : Master's Degree

Preferred : None

Licensure

Required : LCSW / LMHC license active in Florida

Preferred : None

Must haves : Pediatrics - Minimum of 2 years

Nice to haves : Bilingual Spanish / English preferred, Experience with electronic medical records, Basic Excel skills

Disqualifiers : Non licensure

Performance indicators : Caseload compliance

Best vs. average :

Technology including Microsoft and EMR
