Key Responsibilities
Assess member needs across various health and social domains and oversee the development of client care plans
Connect clients to necessary social services and advocate on their behalf with healthcare professionals
Conduct outreach activities, evaluate client progress, and maintain accurate documentation in health records
Required Qualifications
2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
Experience working in a remote setting
Working knowledge of government and community resources related to social determinants of health
General computer skills and familiarity with Google Workspace and MS Office
Bilingual (English / Spanish) preferred
