Social Worker, Hospice & Palliative Care – Full-Time | Central Wisconsin A hospital near La Crosse, WI is seeking a compassionate Social Worker to join their Hospice & Palliative Care team! Social Worker, Hospice & Palliative Care Care Schedule : Full-Time | Direct Hire Monday – Friday | 8 : 00 am – 4 : 00 pm Social Worker, Hospice & Palliative Care Responsibilities : Provide psychosocial assessments, counseling, and support for patients and families Collaborate with an interdisciplinary team to develop and update individualized care plans Educate patients, caregivers, and families on palliative care, comfort measures, and the dying process Maintain accurate documentation, uphold ethical / legal standards, and participate in team meetings and quality initiatives Social Worker, Hospice & Palliative Care Qualifications : MSW from an accredited School of Social Work Wisconsin Certificate from the Department of Regulations & Licensing Examination Board of Family Support Coordinators Minimum of 3 years of clinical experience Completion (or agreement to complete) hospice / volunteer training Why You’ll Love the Area : Experience local festivals and farm-fresh markets Surrounded by lakes, rivers, and trails for kayaking, hiking, fishing, and camping Easy access to La Crosse and Wisconsin Dells for shopping, dining, and entertainment Affordable living in a strong, family-friendly community
