A company is looking for a Care Manager.

Key Responsibilities

Develops and facilitates complex care management activities for mental and behavioral health needs

Evaluates member needs and develops care plans through phone or in-home visits

Coordinates care among members, families, and care provider teams while monitoring member status

Required Qualifications

Master's degree in Behavioral Health, Social Work, or an accredited nursing degree

Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN
