Job Title : Registered Nurse

License : Must have license in hand

Certifications : CPR, BLS

Must-Have : Proficient in HomeCare HomeBase EMR and OASIS, used within the last 6 months or required retraining

Job Description :

• Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies.
• Responsible for supervising and evaluating Home Health Aides and Licensed Practical / Vocational Nurses.
• Conduct home visits and attend periodic office meetings.
• Maintain reliable transportation with a current driver's license and agency-required liability insurance.
• Serve 5-10 patients per day, averaging 6-8 productivity points.
• Handle various cases including Start of Care, Wound Care, and Follow-up Visits.
• Must be willing to travel up to 75 miles from the home clinic, with mileage reimbursed using the IRS standard rate.
• Participate in on-call rotations, generally one night per week and one weekend per month.
• Complete all documentation within 24 hours of patient appointments.
• Be adaptable to floating to nearby clinics if patient census is low to meet financial needs.
• Comply with dress code requirements and maintain proficiency in required electronic medical record systems.
