JOB DESCRIPTION

Department : Intensive Care Unit

Shift Timings : 18 : 00 - 06 : 30 Hours Monday to Friday 3 days / 12 hours

Per Week : 36 Hours

13-week contract; 36 HPW minimum; every other weekend; holiday / hard-to-fill shift TBD by unit need & contract dates.

Roles & Responsibilities

• Implement and monitor patient care plans. Monitor, record and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient's discharge planning needs with members of the healthcare team.
