Health Educator Coordinator
Description :
The Health Educator Coordinator will be responsible for providing and supporting patient education on disease prevention and self-management, health promotion and wellness.
This role will be critical in the successful implementation of health education programs, and collaboration with the care team and community partners to improve health outcomes and quality.
The Health Educator will participate in all aspects of the program including program development, implementation, patient outreach and engagement, and evaluation.
Essential Duties & Responsibilities
- Actively engage in development of health education programming through ongoing literature review to remain current on successful interventions of chronic disease management education approaches.
- Assist in the development and adaptation of health education materials in a variety of formats, such as print, social media, group workshops
- Develop a calendar of varying health education opportunities that meets the diverse needs of our patient population including various styles of delivery, time of programming and language needs
- Provide ongoing outreach efforts to identified panels of patients. Assess patients’ readiness for change and engagement in education programming utilizing motivational interviewing skills.
- Coordinate enrollment in disease prevention and self-management education programs. Collaborate with the care team and document all outreach and enrollment efforts in the Epic Medical Record and appropriate excel spreadsheets.
Monitor enrollment numbers and attendance records.
- Coordinate and deliver health education sessions including but not limited to nutrition education, cooking classes, supermarket tours.
- Coordinate and facilitate opportunities to be physically active. Lead community-based walking programs for adults and youth.
Support 5K training programs.
- Develop database of community-based lifestyle management resources and opportunities including ways to be active and manage stress.
- Assess education needs from patients and communities, maintains accurate records of education activities, collects evaluation data, and prepare content for of project reports
- Promote compliance with clinical intervention and monitoring. Reinforce importance of Primary Care visits through reminder calls, texts and assistance in scheduling follow up appointments as needed.
- Assist in planning, developing, and implementing community health education activities
- Develop collaborations with community organizations. Provide community-based health education sessions in partnership with neighborhood schools and community organizations
- Collaborate closely with community partners to strengthen relationships to meet program goals and expand health education and lifestyle management opportunities
- Participate in community events including health fairs and other events to promote positive health practices in the community
- Work closely with Quality and Let’s Get Movin Departments and support in special events and projects as requested
- Support writing grant proposals, maintain grant tracking as determine by award and in coordination with Grants Department
- Maintain excellent punctuality and attendance
- Promote a sense of team work through demonstration of self-
- direction and self-motivation. Solve problems independently or knows when to seek consultation.
- Adheres to all EBNHC and departmental policies and procedures
- Perform other related duties.
EDUCATION :
Minimum Bachelor Degree, or equivalent experience in Health Education, Public Health, Behavioral Science or related field.
EXPERIENCE :
Minimum 1-3 years’ experience working in a health-related field, preferably in a culturally and linguistically diverse community
SKILLS / ABILITIES :
- Bilingual Spanish and English required.
- Excellent communication and / or counseling skills.
- Proficient computer skills in Excel and MS Word.
- Certified Health Education Specialist (CHES) or other health education training, preferred but not required.
- Travel to different sites within East Boston area may be required.
- May need to work occasional evenings or weekends based on program activities.
