Job Description
RemX has an exciting opportunity for a Case Management RN in West Los Angeles! This is a great opportunity to work for a large Healthcare Facility.
Length : 3 months with potential for extension
Schedule : 8 / 8 : 30 to 5 : 00 / 5 : 00p, no weekends
Title : Case Management RN
Job Responsibilities :
- Responsible for covering outpatient palliative care. Will be cross-trained for geriatric memory care.
- The outpatient palliative care panel has about 800-1000 patients (high volume).
- Will provide outreach calls to members, support flu and medication management, symptom management, escalate issues to physicians.
- Collaborate with social worker for any psycho-social needs
- Responsible for managing physicians’ emails from patients.
- Follow up with members after physician consultations to make sure patient understands what was discussed or the instructions given.
Required :
- Min 1-3+ years RN work experience
- Must have case management experience
- Ability to fully understand the total needs of the patient, their family & social-economic factors that play into the patients issues / symptoms
- Ability to triage and dive deep in finding out the root cause of the patient’s symptoms; know what questions to ask to get to the root cause
- Active BLS by AHA
- Active FLS
Preferred :
- Case Management Certification
- Palliative or hospice care exp
