Job Purpose

We are seeking a compassionate and dedicated Nurse Practitioner to join our interdisciplinary team within the Program of All-Inclusive Care for the Elderly (PACE). The Nurse Practitioner (NP) will provide primary and specialty care services to PACE participants, focusing on prevention, wellness, and chronic disease management. The NP will work collaboratively with an interdisciplinary team (IDT) to develop and implement individualized care plans, ensuring high-quality and person-centered care.

Duties and Responsibilities

• Participant Care :

Conduct comprehensive assessments of participants, including physical exams, health histories, and functional status evaluations.

Actively participate in IDT meetings to review participant care plans and progress.

Coordinate transitions of care, including hospital admissions, discharges, and specialist referrals.

Maintain accurate and timely documentation in accordance with program policies and regulatory requirements.

Qualifications

Education :

Current and unrestricted Nurse Practitioner license in [State].
