Recognizing its excellence in nursing care, Barnes-Jewish Hospital was the first adult hospital in Missouri to be certified as a Magnet Hospital by the American Nurses Credentialing Center.

The CM Services reports through the division of Patient Care Services lead by the Chief Nursing Executive, Vice President of Patient Care Services.

The CM Service is comprised of three departments : Case Management, Utilization Management, and Social Work. Through the Triad Model of Care Delivery, the objectives of the CM Services are for all patients to have access to healthcare through the BJH continuum and appropriate, safe discharge plans.

The SW department is open 24 hours / day while the CM department is open 20 hours / day. Both departments provide coverage throughout the hospital including the Emergency Department, Outpatient Clinics, Registration sites, and Health Information Management.

The Service employs registered nurses, social workers, a license practical nurse, a customer service representative, and administrative assistants.

Preferred Qualifications Role Purpose Responsible for the management direction and coordination of Case Management, components of the Care Coordination Department.

Plans, directs and implements all services related to case coordination, concurrent review, and utilization management programs, ensuring compliance with TJC, state and other regulatory agencies.

May be responsible for per-certification and social services. Responsibilities Manages designated aspects of the case coordination, precertification, clinical documentation, and concurrent review programs to ensure maximal reimbursement, prevent financial loss to the hospital, and to minimize patient liability.

Manages individual(s) including but not limited to : hires, trains, assigns work, manages & evaluates performance, conducts professional development plans.

Ensures that the productivity and actions of that group meet / support the overall operational goals of the department as established by department leadership.

Develops and adheres to departmental staffing, revenue and / or expense budgets. Responds to changes in the business which may affect the ability to achieve the budgets goals, grant expectations.

Makes adjustments in programs to support the needs of the target patient population.
