• 8 am - 5 pm schedule with minimal call shared among all state providers
• RN call center handles calls, only urgent matters reach physicians
• Value-based care experience with HEDIS measures and HCC coding preferred
• Work with complex geriatric patients aged 65+ in integrated care model
• Collaborative team with 2 very strong advanced practice providers
• eClinicalWorks experience preferred for top pay scale consideration
• Board-certified or board-eligible in geriatric medicine required
• Spanish fluency a plus but not required for this urgent position
