Job Description Summary :

Under the general supervision of the Director, and Clinical Coordinator, and responsible for providing hygienic care and assigned procedures for assigned patients based on age-specific needs. The employee is subject to call back and overtime as required by the hospital.

Responsibilities

• Performs direct and indirect patient care responsibilities as assigned by the case manager or Clinical Coordinator.
• Ensures patient safety while performing delegated tasks.
• Performs related responsibilities as required or directed.
• Provides emotional support to families of terminal patients.
• Obtain vital signs as delegated.
• Perform baths, linen changes, shampoos, shaves, mouth care, and nail care.
• Assists patients with a transfer in the home as delegated and if included in the care plan.
• Chart and turn in notes timely according to agency policy.
• Maintain POSITIVE HELPFUL ATTITUDE.
• Uses AIDET on a daily basis.
• Communicate all pertinent information to the case manager.

Qualifications

EDUCATION : High school diploma or the equivalent required.

LICENSURE, REGISTRATION, CERTIFICATION : BLS certification must be obtained within the orientation period, and certification must be kept current while in this position. Must have a valid driver's license and auto insurance.

EXPERIENCE : Current skills and knowledge obtained from the completion of an approved Nursing Assistant program or certification preferred.
