Arosa strongly prefers a candidate who lives within 30-40 mins of office/service area. Candidate must be willing to drive to/from/around this area daily between 8a-5p M-F. Service area includes but is not limited to the following towns: Watertown, Waltham, Newton, Needham, Wellesley, Weston, Dedham, Brookline, Arlington, Lexington, Belmont, Medford, Cambridge, Somerville, Brighton, Allston, and Boston (including Charlestown, East Boston, South Boston, North End, South End, Back Bay, Dorchester, West Roxbury, Jamaica Plain).
Candidate also needs to be open to a regional on-call rotation roughly every 6 weeks.

Care Management & Professional Outreach:

  • Work directly with clients and their family members; provide support for home care services and community-based care management services. Conduct professional assessments to understand our client's physical, emotional, social, and spiritual needs, develop a care plan that affords a sense of stability, clarity, and peace of mind, and manage the plan.
  • Be our face in the community: Participate on local committees, task forces, and boards. Tasks may include education / presenting to professional groups; raising consciousness and helping professionals learn how we can help them help their clients/patients; setting up meetings with professionals, getting to know them, figuring out what they need/want, and helping create solutions.

It takes a special professional, who is as comfortable with older clients and their families, as they are with our client’s satellite support providers, such as Trust Officers, Attorneys, Physicians, and other elite professional service providers. We are looking for someone who has confidence in the value of Care Management services. A teammate who conveys, "I'm so glad you called us, as I know I can truly help you.”

Knowledge and Other Requirements:

  • Educational Minimum Requirement: a Bachelor’s Degree. Preferred candidate: Master’s Degree in a healthcare-related field.
  • Ability to interface effectively and professionally with client support, including family, physicians, attorneys, etc.
  • Ability to work in a team environment required.
  • Acceptable candidate: Bachelor’s or Master’s Degree in Social Work, Nursing, Gerontology, Psychology, Health and Human Services, Occupational Therapy, Licensure in nursing (LVN/LPN or RN), or other healthcare-related fields.
  • Preferred: A minimum of two (2) years experience in care management, experience working with the aging population preferred; other professional experience may substitute.
  • Excellent written and verbal communication skills required.
  • PC literacy required.
  • Valid driver’s license and proof of current auto insurance required. They must be willing to transport clients in their own vehicle. Mileage reimbursement included.
  • Strongly prefer a candidate who lives within 30-40 mins of our office/service area. They must be willing to drive to/from/around this area daily between 8a-5p M-F.
  • Office address: 50 Hunt St Watertown MA
  • Service area includes but is not limited to the following towns: Watertown, Waltham, Newton, Needham, Wellesley, Weston, Dedham, Brookline, Arlington, Lexington, Belmont, Medford and Boston, etc.
  • Must be willing to be part of an on call rotation. As of now, CMs are on call every 6 weeks. On call includes M-F from 5p-9p and weekends from 8a-9p. They will be paid a stipend for on call.
