We are hiring a full-time case manager in Salt Lake City, Utah!!
The PROUD case manager serves clients that are in recovery from opiate use disorder to help them promote financial self-sufficiency, housing stability and long-term recovery upon completing First Step House's residential treatment program. The goals of this program are to (1) provide housing case management services as these clients transition from residential treatment to a sober living setting (2) develop individualized service plans with clients that focus on finding permanent housing and (3) connect clients with community resources and sober supports. The role of the PROUD case manager is to assist the recovery residence teams with client check-ins and daily facility checks, complete individualized case plans with clients, assist clients with finding permanent housing options in the community and provide additional case management services for up to two years to help them remain stable in permanent housing.
