Description
We are seeking a dedicated and detail-oriented Clinical Care Coordinator to join our healthcare team. In this vital role, you will be responsible for the coordination of patient care activities to ensure that our patients receive the highest quality of care throughout the healthcare continuum. You will act as a liaison between patients, families, and healthcare providers, facilitating communication and collaboration to improve patient outcomes. Your role will involve assessing patient needs, developing care plans, and monitoring the progress of patients while ensuring compliance with health standards and regulations. You will use your strong organizational skills and clinical knowledge to manage patient information, schedule appointments, and follow up on treatment plans. This position requires a compassionate, patient-centered approach to care, as you will often support our patients through complex medical situations. Responsibilities

- Participate in quality improvement initiatives aimed at enhancing patient care and operational efficiency.
