Deer Meadows Home Health & Support Services is seeking a dedicated and compassionate Part-Time Physical Therapist Assistant (PTA) to provide exceptional home health services to patients throughout the community. If you enjoy working independently, building meaningful patient relationships, and helping individuals regain strength, mobility, and independence, we'd love to hear from you!
Provide skilled physical therapy treatments in patients' homes under the direction and supervision of a licensed Physical Therapist.
Carry out individualized treatment plans designed to improve mobility, balance, strength, endurance, and functional independence.
Educate patients and caregivers on home exercise programs, safety techniques, and fall prevention.
Monitor patient progress and communicate changes to the supervising Physical Therapist.
Complete accurate and timely clinical documentation using Homebase Home Care EMR.
Collaborate with physicians, nurses, and the interdisciplinary care team to ensure high-quality patient care.
Maintain compliance with agency policies and all applicable state and federal regulations.
Current Physical Therapist Assistant (PTA) license in PA
Graduate of an accredited Physical Therapist Assistant program.
Home health experience preferred, but not required.
Experience with Homebase Home Care EMR is a plus.
Excellent communication, organization, and time management skills.
Current CPR certification (or ability to obtain).
Valid driver's license, reliable transportation, and current auto insurance.
