Summary :

The Health Coordinator facilitates patient continuity of care with the healthcare team. This position is responsible for utilizing available resources to ensure patients / families receive care that is continuous, comprehensive, and coordinated across the care continuum. The Health Coordinator works cooperatively with providers and personnel to carry out the goals and objectives of ambulatory care management according to established policies and procedures.

Responsibilities :

  • Outreach to patients to recommend needed services
  • Act as liaison between health services and our community to facilitate patient access to quality service and healthcare delivery
  • Facilitate communication and patient / caregiver empowerment in interactions with healthcare systems
  • Assist patients to improve health outcomes, and to reduce unnecessary ED utilization
  • Regularly coordinate and communicate with Care Team members on all care plan activities, including referrals, transition care planning, and follow-up tracking
  • Review Disease Management Registries to identify patients at assigned clinics with gaps in care which includes but is not limited to pediatric wellness, preventative, chronic illness, and sick visits
  • Notify RN Care Managers, primary care physicians and clinical office staff of issues pertinent to patient care
  • Work directly with assigned clinics to determine outstanding needs and develop rapport with physicians and staff to assure seamless integration of activities with clinic Operations
  • Follow all HIPAA rules and regulations when providing PHI to internal and external providers. This includes but is not limited to the verification of phone and fax numbers, confirmation of appropriate receipt and appropriate identification of person with whom information is being shared
  • Document appropriately in selected tools using correct format, grammar, punctuation and spelling. Communicate and document conversations with internal and external providers. Inform team members of denials and potential denials so appropriate action can be taken
  • Other tasks and assignments as assigned or requested

Other information : WORK EXPERIENCE

  • At least 2 years experience in a healthcare setting required
  • At least 4 years experience at a Pediatric / Family Practice or on a Pediatric Med / Surg floor preferred
  • EDUCATION

  • High school diploma or equivalent required
  • LICENSES AND CERTIFICATIONS

  • Effective 1 / 1 / 2023, Medical Assistant certification is required for current employees in this role by 12 / 31 / 2023; MA certification is required for new hires within 1 year of hire required
  • Another related certification preferred
