About The Role
Addison Group is hiring an experienced and passionate Community Health Worker (Bilingual – Spanish) on behalf of our valued healthcare partner in Austin, Texas. This is a full-time, onsite opportunity serving an innovative care team focused on improving patient outcomes, health equity, and community engagement for Medicare-eligible adults.
Key Responsibilities
As a Community Health Worker, you will be a vital connection between clients, families, and community resources — advocating for older adults, bridging language and cultural gaps, and helping clients navigate complex care systems. Key responsibilities include :
Provide community-based support and culturally competent outreach to Spanish-speaking older adults and caregivers.
Conduct in-person visits in clinical settings, health education, and follow-ups to assess needs and progress.
Help clients understand and access medical appointments, benefits, and social services.
Build strong relationships with clients, families, and multidisciplinary care teams.
Document client interactions and outcomes in electronic health records.
Connect clients to transportation, food, housing, and other essential community support.
Act as a trusted advocate to promote engagement, self-sufficiency, and optimal health outcomes.
Requirements
Experience :
Bilingual proficiency in Spanish and English (verbal and written).
Experience working in medical clinical settings.
Experience working in community health, social services, care coordination, or related fields.
Strong interpersonal and communication skills with older adults and family caregivers.
Comfort engaging clients in their homes and community settings.
Education
High School Diploma OR Equivalent (required)
Licensed Clinical Social Worker (LCSW) (preferred)
Bachelor's in Social Work (preferred)
Master's of Social Work (preferred)
Skills
Excellent written and verbal communication skills
Strong analytical and problem-solving abilities
High attention to detail and accuracy
Confident in communicating with patients, providers, staff, and leadership
Positive, go-getter attitude and willingness to jump in where needed
