Job Summary and Qualifications

The role of the RN Case Manager is to assess each assigned patient referral to identify discharge planning needs, and collaborates with patient, family, significant other, and members of the health care team to develop and implement comprehensive plan of care and discharge plans. Balances individual clinical and psychosocial patient / family needs with the efficient and cost-effective utilization of resources, while promoting quality outcomes through the continuum of care. Reviews and performs utilization review functions on each assigned patient per hospital policy and procedure.

Minimum of 3 years of medical, surgical or critical care experience.

Previous emergency, pre-hospital or critical care preferred.
