Deliver safe, patient-centered nursing care in the home Serve as the primary case manager, coordinating care with the interdisciplinary team Develop and implement individualized plans of care Provide patient and caregiver education, support, and disease management teaching Ensure accurate, timely documentation (documentation time built into the home visit) Supervise and provide oversight of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patient's plan of care Maintain compliance with all infection control and regulatory standards Stay current with clinical best practices Current NYS Registered Nurse license (from an accredited program) BSN preferred (tuition reimbursement available!) As we continue to expand our clinical programs to meet increasing community needs, we are seeking RN Case Managers to join our teams in Onondaga, Oneida, Oswego, Jefferson, or Cayuga Counties!
Up to $15,000 Sign-On Bonus

At Nascentia Health, you'll provide skilled, compassionate care to adults with chronic, acute, or complex medical needs in the comfort of their homes, helping patients maintain independence, improve quality of life, and feel supported in their own environment. We believe in listening to our nurses, supporting them, and setting them up for success every day. Home visits are scheduled Monday-Friday, 8 : 00 AM-4 : 30 PM, with no on-call requirement, and a one weekend a month rotation. This Full-Time position offers flexible scheduling and a full benefits package!
